You will be based in a diverse team and manage a caseload of patients from the City South area. Your role will involve being a Key Worker, planning, facilitating and reviewing patient-centred care, involving their family, carers and external agencies.

The ideal candidate will be a team player, able to manage a caseload and work autonomously, contribute their ideas to the MDT and build therapeutic rapport with patients. You will support on duty, as well as supporting junior staff and students.

The Local Mental Health Teams provide mental health services for people aged 18 to 65 years across Nottingham City, Nottinghamshire County and Bassetlaw. There are eleven LMHTs, each of which includes the following specialist staff: Mental Health Nurses, Occupational Therapists, Psychologists, Psychiatrists, Social Workers, Community Support Workers, Peer Support Workers and Employment Specialists.

Our LMHT brings together a variety of services including Assertive Outreach, Community Rehabilitation, Community Assessment Treatment Services, City Recovery Service, Social Inclusion and Wellbeing, Community Occupational Therapy Service and Medical Services. We help patients and their families to cope with periods of severe mental illness and assess, plan and implement a personal path of recovery for each patient. We work with people who have a wide range of mental health difficulties. Some may require short-term intervention, and sometimes longer-term care plans will be put in place.

#TeamNottsHC comprises over 11,000 dedicated colleagues who #MakeADifference every day. We deliver intellectual disability, mental health, community health, forensic, and offender healthcare services across Nottinghamshire, Leicestershire, Lincolnshire, and South Yorkshire. Our care is provided from over 200 sites, spanning community locations, acute settings, and secure environments, including prisons.

We are one of the largest mental health and community Trusts in the East Midlands and one of Nottinghamshire's biggest employers. We also host national and regional services, such as the National High Secure Deaf Service and the Nottingham Centre for Transgender Health.

We offer a variety of employee-led staff networks, including Equality, Diversity, and Inclusion (EDI) groups, the Green Champions network, the Freedom to Speak Up network, the Health and Wellbeing Champions network, and the Menopause Champions network. These networks play a vital role in supporting our diverse workforce and promoting a culture of inclusivity.

The health and wellbeing of our colleagues is a top priority. We invest significantly in this through our in-house occupational health and staff counselling services, supported by a dedicated Health and Wellbeing team.

The Trust is committed to reducing its carbon emissions, with a specialised Energy and Environmental team working to ensure compliance with environmental legislation, enhance our environmental performance, and achieve our net-zero commitment.

You will need to be able to engage proactively with individuals using the principles of recovery to provide effective mental healthcare for patients across the mental health spectrum, alongside care coordination for some patients under the Care Programme Approach. You will also have the opportunity to contribute to the development of the service and develop your own skills and attributes.

Duties will include involvement in managing a medication clinic, case management of clients with complex mental health needs, utilising professional core skills, working with family and friends. The role will include involvement in the development and promotion of the City South Local Mental Health Team.
