Children and Young Persons Practitioner in Nottingham

Job Purpose

To work as part of an integrated Children's Public Health 0 -19 Nursing team and support in the delivery of the Healthy Child Programme to children aged 0 to 19 years old in a variety of community settings.

Will participate in the development of a range of health promotion initiatives which aim towards improving the physical, social, and emotional well-being of children, young people and families.

Dimensions

The post holder will work as part of a geographical, skill mix team to support the delivery of high quality public health services to children, young people, and their families in Nottingham city.

Organise and complete delegated work effectively.

Undertake other duties as may be required for maintenance of an effective and efficient service.

Key Responsibilities

• Quality and Performance Undertake Healthy Child Programme, developmental reviews and public health interventions delegated by the Specialist Community Public Health Nurse.
• Undertake specific public health programmes of care, such as behaviour/parenting support, sleep management support, continence, healthy lifestyle and speech and language assessments in a variety of settings e. g. home, clinics, childrens centres, and schools.
• Be able to facilitate small groups to deliver health promotion messages e. g. weaning, oral health.
• Providing routine information to children, young people and families which requires persuasive skills, particularly when there are barriers to understanding.
• Receive information from clients who may have difficulty conveying their views and ensure provision is made to facilitate their ability to achieve this e. g. when English is not the first language, SEND.
• To be able to assess the needs of families under the direction of a Specialist Community Public Health Nurse or Community Public Health Nurse using the relevant assessment tools
• To use evidence-based programmes of care in line with City Care policies and guidelines to deliver safe and effective care.
• Undertake targeted health promotion initiatives in a variety of settings e. g. homes, clinics, schools.
• Undertake any other duties which may be reasonably required within the remit of the role.
• Ability to contribute towards achieving key performance targets.
• To contribute to the writing of reports, feedback to the caseload holder and attend meetings/ reviews under supervision to support targeted families.
• Maintain an accurate diary in accordance with the lone working policy and adhere to agile working guidance.
• Work in a cost-effective manner.
• Contribute to processes to maintain stock control and ordering of resources required to undertake role.
• Actively contribute to maintenance and security of equipment used within the team.
• Contribute to activities aimed at increasing service efficiencies e. g., reduced printing and signposting to alternative resources.
• Professional Work in accordance with Nottingham City Local Safeguarding Childrens Board procedures.
• Provide and receive routine information, orally, in writing or electronically in line with Nottingham City Care policy and abiding by legal requirements.
• Ensure clear, accurate and up to date recording of information on Systm One as per local record keeping policy.
• Complete all appropriate records/documents in a clear, accurate and timely way, participating in reporting processes as necessary.
• Ability to organise and prioritise own workload under the supervision of senior members of the team.
• Observe confidentiality at all times.
• To demonstrate empathy, compassion, and ability to reassure.
• Be sensitive to the needs of children, young people and their carers.
• Where communications/barriers are evident identify appropriate resources to optimise understanding for parents and young people.
• Participate in the induction/training of new staff.
• Provide shadowing opportunities to students on placement with the service.
• Delegate work to appropriate support staff according to their remit.
• Offer leadership to children and young peoples support workers in the undertaking of their roles e. g. shadowing opportunities and undertaking of clinical supervision/appraisals.

To attend mandatory and role specific training sessions as defined by Nottingham City Care Partnership To understand City Care policies and procedures related to your clinical role.

To receive annual review and personal development planning to promote achievement of individual, team and organisation objectives.
