The Bladder and Bowel Health Service delivers high-quality, patient-centred care across Norfolk. Working independently within patients’ homes, care homes, clinics, and community settings, the post holder assesses, manages, and supports individuals with bladder and bowel health needs, promoting continence, independence, dignity, and quality of life.
The role involves undertaking comprehensive assessments, developing and reviewing care plans, providing treatment and product recommendations, and delivering specialist education and support to patients, carers, care home staff, and multidisciplinary colleagues. The post holder works collaboratively within an integrated healthcare team, contributing to service development, clinical governance, audit, and evidence-based practice while maintaining professional standards in line with the NMC Code.
In addition, the nurse provides supervision and support to Band 3 and 4 staff, students, and learners, ensuring safe, effective, and compassionate care. The role requires excellent communication, clinical assessment, organisational, and leadership skills, alongside a commitment to continuous professional development and delivering community-based healthcare services in a dynamic and evolving environment.
We are looking for someone who can:
- Participate as a member of the Bladder and Bowel Health Team and be accountable for ensuring that standards outlined in the Norfolk wide strategy for continence are maintained.
- Provide a high standard of care to patients within their own homes, care homes, the travelling community, and in clinics, by promoting continence, managing incontinence, and promoting their independence while respecting their privacy and that of their families.
The main responsibilities include:
- Work as a member of the integrated Bladder and Bowel Health Service, in conjunction with other professionals, to promote a holistic model of care in line with national best practice.
- Promotion of continence, first level surveillance and assessment of vulnerable adults.
- Work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
