Nurse - Prevention and Physical Health Programme

Key Responsibilities

  • Prevention and Population Health Support delivery of prevention programmes by:
  • Identifying eligible patients using agreed inclusion criteria
  • Delivering prevention clinics within practices and community settings
  • Supporting population health initiatives
  • Promoting self-management
  • Reducing health inequalities
  • Supporting early identification of long-term conditions
  • Physical Health Assessments Undertake comprehensive nursing assessments including:
  • AF Case Finding
  • Single-lead ECG recordings
  • Pulse rhythm assessment
  • Blood pressure monitoring
  • Cardiovascular risk assessment
  • Recognition of abnormal findings requiring GP review
  • SMI Physical Health Checks Undertake comprehensive annual physical health reviews including:
  • Blood pressure
  • Pulse
  • Oxygen saturation
  • Temperature
  • Height
  • Weight
  • BMI
  • Waist circumference (where appropriate)
  • Venepuncture
  • Lifestyle assessment
  • Smoking assessment
  • Alcohol screening
  • Physical activity assessment

  • Physical health questionnaires

• Clinical Decision Making Working within competence, the nurse will

  • Recognise abnormal clinical findings
