The predicted salary is between 31500 - 38500 £ per year.

A driving licence and access to a vehicle are desirable, as travel between our schemes in Newbury and Thatcham may be required. However, this is not essential, provided candidates can reliably travel to both locations. Mileage expenses are reimbursed in line with company policy.

Role Overview: To deliver high quality person-centred support to customers living in supported accommodation service in accordance with the Person Centred principles, enabling clients to sustain their tenancies by promoting independence, freedom and choice.

Key Responsibilities:

  • Assess referrals and engage with prospective clients using a person-centered approach.
  • Liaise with health professionals, other agencies and colleagues as appropriate.
  • Forge and maintain links with partner agencies and other key stakeholders as appropriate to ensure that good communication and co-operation is maintained.
  • Ensure that prospective and current clients are fully aware of the support available, their financial obligations, rights and responsibilities under their tenancy/occupation arrangements and all related procedures.
  • Agree and review client Independent Living Plans and Risk Assessments on a regular basis, deliver contracted hours of support to achieve the required outcomes.
  • Report any concerns relating to safeguarding of vulnerable people using the correct procedures, attending case conferences and other meetings as required.
  • High level of relationship skills: empathy, mutual acceptance, supportive and constructive challenge; positive expectation for the future.

More about you:

  • A working knowledge and understanding of how to support people in achieving and maintaining independence is essential.
  • Experience of providing support to diverse client groups.
  • Experience of working with clients experiencing substance misuse and mental health.
  • Educated to GCSE level or equivalent in Maths and English (Grades A*- C).
  • Ability to work under own initiative.
  • Ability to network and build strong relationships with partner agencies, community and key stakeholders.
  • Ability to use IT packages and management information systems.
  • Ability to deal with distressed and aggressive clients.
  • Driving Licence and use of own transport desirable.
