The Neurology Specialist Nurse provides expert assessment, support, education, and clinical management for people living with long-term neurological conditions, including Multiple Sclerosis (MS), Parkinson's Disease (PD), Motor Neurone Disease (MND), and Huntington's Disease (HD).

Working autonomously across community and clinic settings, the role coordinates care between patients, carers, healthcare professionals, and voluntary agencies to ensure high-quality, person-centred care.

The post holder manages a specialist caseload, monitors treatment effectiveness, provides specialist advice, delivers education to patients and professionals, and contributes to service development, audit, and quality improvement within the Community Neurology Service.

  • Provides a specialist resource within the specialist nursing role for individuals with Multiple Sclerosis (MS), Parkinson’s Disease (PD), Motor Neurone Disease (MND) or Huntington’s Disease (HD), their carers, members of the general public, health professionals, statutory and voluntary bodies.
  • Works within the Central Norfolk Neurology Service and acts as a conduit in the care of individuals with the identified Neurological conditions, between a variety of services and settings.
  • Develops the knowledge of individuals with a long-term neurological condition, their relatives and carers, members of the general public, health professionals, statutory and voluntary bodies about the management and symptoms associated with these diagnoses.

  • Actively facilitates and participates in improving and enhancing thedelivery of the care given to individuals with a long-term neurological condition, at a local, regional and national level.
  • To triage nurse specialist referrals for individuals with a long-term neurological condition, who have a GP within Central Norfolk.
  • To provide assessment, monitoring, support and education for people with the identified neurological conditions using recognised assessment tools, evaluate findings and develop a goal plan.
  • To monitor drug therapies and recommend changes as appropriate in line with professional guidelines (Nursing and Midwifery Council) and local and national guidelines.
