The Multiple Sclerosis (MS) Care Co-ordinator will play a key role in supporting the delivery of a responsive, efficient and patient-centred MS service. Working as part of the multidisciplinary team, the post holder will co-ordinate patient pathways, facilitate communication between patients and healthcare professionals, manage administrative and clinical support processes, support disease-modifying therapy (DMT) monitoring pathways, and act as a key point of contact for people living with MS.
The role is designed to release specialist nursing time from routine coordination and administrative functions, enabling the MS Clinical Nurse Specialists to focus on complex clinical assessment, treatment decision-making, newly diagnosed patients and advanced MS care. The post holder will work within agreed protocols and under the supervision of the MS CNS team.
- Point of contact for people living with Multiple Sclerosis (MS), their carers and healthcare professionals, coordinating care across acute, community and primary care services.
- Manage referrals, appointments, annual reviews and follow-up care, ensuring patients receive timely access to appropriate services.
- Support Disease Modifying Therapy (DMT) monitoring by tracking investigations, maintaining accurate records and databases, and coordinating follow-up actions.
- Provide non-clinical information, guidance and signposting to support patient engagement and self-management.
- Facilitate effective communication between consultants, specialist nurses, GPs, therapists and external providers, supporting multidisciplinary working and continuity of care.
- Maintain electronic patient records, produce activity reports and support audits and service improvement projects.
- Manage administrative processes, waiting lists and correspondence while ensuring compliance with confidentiality, information governance and organisational policies.
- Work within agreed protocols, escalating clinical concerns appropriately and contributing to a positive team culture.
