Cheshire and Wirral Partnership NHS Foundation Trust is seeking a Part-time Practice Nurse (Band 5/6) to work 16–24 hours across 2–3 days. The post involves ongoing assessment of care needs, development and evaluation of care programmes, and maintaining high nursing standards within the practice.
The candidate will have 1st level RN registration with at least 2 years’ Band 5+ experience, with chronic disease management, diabetes care and cervical screening knowledge.
