The post holder will be working in a Complex Psychosis Team within the Assertive Outreach Pathway, taking responsibility to assess, plan and coordinate the care plan and treatment of a defined caseload of service users with complex needs related to psychotic illnesses.

The Complex Psychosis Teams are a multidisciplinary, community-based service responsible for providing ongoing holistic assessments, treatment and support to clients with severe and enduring mental illness living in the community. The service will enable patients with severe mental health problems to be supported and cared for in the community in the least restrictive setting through an assertive, personalised and integrated approach to care and treatment.

The service will provide an intensive community-based mental health service delivered in service users’ home environments and in supported living accommodation or other locations agreed with the service user utilising “A whole system approach” to recovery from mental ill health. The approach maximises an individual’s quality of life and social inclusion by supporting engagement, encouraging their skills, promoting independence and autonomy in order to give them hope for the future which leads to successful community living through appropriate support.
