Conwy County Borough Council is seeking a motivated Assessing and Reviewing Officer to join the Mental Wellness Team within the Vulnerable People’s Service on a full-time, hybrid basis in Colwyn Bay. You will work with individuals, their families and carers, including those recently discharged, to support recovery, reablement and outcome-based planning. The role includes carrying out Section 117 reviews under the Mental Health Act and collaborating with Community Mental Health Teams.
