We are recruiting a Band 5 Community Nurse to deliver high-quality, person-centred nursing care in patients' homes, following national and local policies and the NMC Code. You will work with patients, families, GPs and a multidisciplinary team to assess needs, plan and deliver care, promote independence and support positive health outcomes.
