An exciting opportunity has arisen for an enthusiastic and motivated Band 6 Physiotherapist to join our Brent Community Rehabilitation Team for a 6-month fixed-term contract, based at Wembley Centre for Health and Care. Please note: Due to the fixed-term nature of this post, it is not eligible for Skilled Worker visa sponsorship.

As part of our friendly and supportive multidisciplinary team, you will deliver high-quality, patient-centred rehabilitation within the community. You will manage a varied caseload of patients, supporting individuals to maximise their independence and achieve their rehabilitation goals in their own homes.

The team forms part of the wider Integrated Community Falls and Neuro Rehabilitation Service and works collaboratively alongside District Nursing and Integrated Care Coordination Services, providing truly integrated care across Brent.

We are looking for a physiotherapist with experience in rehabilitation, community services and/or older adult care who enjoys working autonomously while collaborating closely with the wider multidisciplinary team. The role includes participation in a 7-day rota.

Undertake comprehensive specialist physiotherapy assessments independently and as part of a multidisciplinary team within patients' homes and community settings. Assess, diagnose and manage a diverse caseload of patients presenting with a range of complex conditions, including musculoskeletal, neurological and frailty-related conditions. Develop, implement and evaluate evidence-based, person-centred treatment and rehabilitation plans, adapting interventions to meet each individual's clinical needs and rehabilitation goals. Work collaboratively with multidisciplinary colleagues, carers and external agencies to promote safe, effective and coordinated care that supports patients to maximise their independence and remain within their own homes wherever possible.
