Are you passionate about helping people live independently and achieve the best possible quality of life? Join our Adult Community Services team and make a real difference to patients with complex health and social care needs. As a Care Coordinator, you'll be a key point of contact for patients, carers, and professionals, ensuring joined‑up support across health, social care, and voluntary services. You'll manage a caseload, coordinate care, and help patients access the right services to maintain their independence and wellbeing.

In this rewarding role, you will:

• Support housebound patients living with long‑term conditions.
• Help reduce avoidable hospital attendances and admissions.
• Respond to referrals and coordinate access to services.
• Provide advice and support to patients, carers, and colleagues.
• Develop personalised plans that promote independence and prevent care crises.
• Empower patients to proactively manage their health and wellbeing.

Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles.
