Patient assessment Clerk and post take administration; writing in patients notes. Undertake patient reviews. Promptly update patients systems. Attend clinics (Acute Medicine). Attend Multi-Disciplinary Team (MDT) meetings. Facilitate daily board round with MDT. Triage to medical teams/acute physicians as appropriate. Participates in ward rounds. Liaising with patient families to discuss patients progress, goals and achievable outcomes. Liaising with patients respecting their right to reach decisions about their treatment, care and wishes for discharge destination.
