Main Duties and Responsibilities
- Independently manage patients with long-term conditions including: Diabetes Asthma COPD Hypertension Cardiovascular disease
- Undertake annual reviews and develop personalised care plans.
- Request and interpret relevant investigations within scope of practice.
- Support patients with self-management and lifestyle modification.
- Cervical Screening
- Perform cervical cytology (smear tests) in accordance with national guidelines.
- Ensure appropriate documentation and follow-up of results.
- General Practice Nursing
- Provide a full range of treatment room services including: Childhood and adult immunisations.
- Travel vaccinations (where trained).
- Wound care and dressings.
- Removal of sutures and clips.
- Ear irrigation (where appropriate).
- ECG recording.
- Venepuncture.
- Blood pressure monitoring.
- NHS Health Checks.
- Health promotion and lifestyle advice.
- Infection prevention and control.
- Stock management and vaccine cold chain management.
- Contribution to annual Influenza Vaccination Programme
- Clinical Responsibilities
- Deliver evidence-based nursing care.
- Maintain accurate electronic clinical records using Systm One.
- Participate in clinical audits and quality improvement initiatives.
- Work within NMC Code and practice policies.
- Recognise and respond appropriately to safeguarding concerns.
- Participate in mandatory training and professional development.
- Team Working
- Work closely with GPs, Nursing Team and the wider multidisciplinary team.
- Support colleagues to deliver excellent patient care.
- Attendance and contribution at practice meetings and service development.
- Adhere to Practice Values and Behaviours on a day-to-day basis.
