Bank Community Diabetes Specialist Nurse
To contact, assess, monitor and support patients/clients and their families ensuring holistic health needs assessment as well as medication compliance, treatment and follow up. Liaise with Endocrinologists, GP's and community staff as required. Organise/attend/participate in diabetes team meetings as required. To advise on treatment plans for the patient. To develop individual care plans in partnership with clients based on a shared assessment of need. To formulate and review/update diabetes policies. Attend in service training courses and study sessions as appropriate to maintain own competencies. Plan, implement and evaluate diabetes training to students and other members of ECCH staff. To act as a source of expert advice regarding diabetes information to patients and other health care professionals. Maintain contemporaneous and accurate written and electronic records in partnership with clients and in accordance with policies and procedures. Take responsibility for management of team members as required. To lead on tasks as delegated by line manager. Write reports and participate in audit on service delivery, as required for manager and ECCH Board. Devise local protocols for specialist working and ensure implementation as required.
