About this role…
We are looking for a Support Worker to join the Alliance North Living Well Centre Team. The Lambeth Living Well Network Alliance supports people in Lambeth experiencing mental illness. Thames Reach works in partnership with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough.
You will:
- Be a capable and enthusiastic support worker
- Be joining the North Living Well Centre, a service which is part of the innovative reshaping of Mental Health services in Lambeth.
- Work directly with clinical staff to manage a caseload of people with Mental Health needs in the Lambeth Community, focusing on resolving practical issues and improving their wellbeing and social integration and supporting them to lead independent lives in the community alongside their GP.
- Work with Staff in the Living Well Centres supporting clients using a mix of brief interventions or longer-term support as needed, delivered by multi-disciplinary teams consisting of support workers, community psychiatric nurses, social workers, occupational therapists, peer mentors and consultants.
