Main responsibilities include:
- To be an integrated team member within the ward team, and Lewisham OT team to address occupational and recovery goals for patients.
- To undertake occupational therapy assessments for a designated caseload, assessing health and well-being from an occupational perspective.
- In collaboration with the multi-disciplinary team and service users, to plan and implement individual or group therapeutic interventions using graded activity to achieve therapeutic goals which support recovery.
- To utilise outcome measures that give clear evidence that occupational therapy has supported individual recovery and social inclusion.
- To apply a high level of understanding of the effect of disability and provide training and advice on lifestyle changes and adaptations to the service users' social and physical environment.
- Where applicable, to assess and prescribe small aids and equipment and recommend adaptations to maximise independence for service users in home and community environments.
- Being active in discharge planning, providing occupationally focussed recommendations and working closely with discharge coordinators, care coordinators, wider involved agencies and families/carers to ensure transition into community is done in the most effective and person centred way.
