An exciting opportunity has arisen for a passionate and enthusiastic Band 3 Health Care Assistant on Jasmine Ward (Tolworth Hospital). Jasmine Ward is a 16-bedded unit that offers care and treatment for male and female patients over 75 years of age. The service provides inpatient care for older people with mental health problems, offering assessment and treatment for those who require hospital care due to the severity of need.
The ward is located in Red Lion Road, Surbiton Tolworth with good local links via underground and bus to both outer London. We are looking for individuals who have a genuine passion for working with elderly patients and are willing to make a difference in their lives. You will be expected to deliver high standards of care alongside our team of nurses, doctors, and therapists.
You will need to be a proactive person with experience working with older adults, providing all aspects of support to meet their psychological and physical health needs, including personal care, physical health monitoring, and emotional support. You will work closely with nurses, doctors, and the wider multidisciplinary team to ensure patients’ needs are met in a dignified and respectful manner.

Key Responsibilities:
- Working under the guidance of a Registered Nurse/ AP/NA/B4 to implement the delivery of quality nursing care.
- Leading on and undertaking activities to ensure that the fundamentals of care are met - nutrition, hydration, hygiene, comfort, emotional and social support.
- Supporting junior staff and leading on providing hands-on care to service users.
- Recognising and valuing service users as individuals, ensuring that care respects equality and diversity.
- Monitoring and recording service user clinical observations and escalating any concerns to a Registered Nurse.
- Implementing the principles of ‘recovery’ by encouraging and supporting service users to engage with healthy lifestyles.
- Facilitating the admission and discharge of service users, ensuring their needs are met.
- Demonstrating an enhanced understanding of Mental Health signs and symptoms.
- Contributing to effective communication processes with appropriate and accurate information to patients, carers, family, friends, and colleagues.
- Demonstrating effective clinical communication and documentation.
