We are seeking enthusiastic and experienced mental health professionals to join our Crisis and Home Treatment Teams as Band 6 Home Treatment Team Practitioners.
This is an exciting opportunity to work within a dynamic multidisciplinary crisis service providing rapid assessment, intensive home treatment and crisis intervention for adults experiencing acute mental health difficulties.
You will play a key role in supporting people to remain safely in their own homes during periods of crisis, preventing unnecessary hospital admissions and facilitating timely discharge from inpatient services.
Working as part of a team-based model, you will provide compassionate, recovery-focused care and support individuals and their families through some of the most challenging times in their lives.
- Undertake rapid mental health and risk assessments in community settings and service users' homes.
- Deliver intensive home treatment and recovery-focused interventions.
- Participate in gatekeeping assessments and decision-making regarding hospital admissions.
- Develop collaborative crisis, relapse prevention and discharge plans.
- Support service users and carers throughout periods of acute mental health crisis.
- Work closely with inpatient wards, community teams, Liaison Psychiatry and external partner agencies.
- Maintain accurate clinical records and ensure compliance with Trust standards.
- Participate in clinical supervision, audit, quality improvement and service development activities.
- Provide mentorship and support to students and newly qualified staff.
- Contribute to the day-to-day running of shifts and multidisciplinary team working.

- The post holder will be a member of the Crisis and Home Treatment Team.
The post holder will also have responsibility of working some weekend and night shifts which may entail undertaking assessments or offering support across geographical borders within South West London.
- The main target group are people with severe mental health problems experiencing mental health crisis.
- The Crisis & Home Treatment Team (CHTT) is a multidisciplinary, community based mental health team that aims to provide a safe and effective home based assessment and treatment service as an alternative to in-patient admission, and for early discharge from the ward in the
- least disruptive and restrictive environment.
It works in conjunction with, and is integrated with, other local service elements.
The workload is team-based and team members do not carry a specific caseload.
Care by the team is for days or a maximum of 6 weeks but not longer.
- The Team is available to provide care and intervention 24 hours per day, 365 days of the year for people who are experiencing a mental health crisis of a nature or degree that would require admission in the absence of the CHTT.
- The SWLSTG Mental Health Trust is required to gate keep all admissions to hospital.
- The key features of the service are that it is accessible, mobile, responds quickly and acts as the gatekeeper to in-patient services and assessments for psychiatric presentations.
It offers care within a bio/psycho/social model with emphasis on symptom management, within a Recovery
- and Wellbeing Model and encouraging the service user (and families) to cope and understand their crisis.
