We are seeking an enthusiastic and motivated Community Specialist Nurse (Diabetes) to join the Community Diabetes Service.

The successful candidate will work as part of a multidisciplinary team to deliver high-quality, evidence-based diabetes care, supporting people with diabetes to effectively manage their condition and achieve improved health outcomes.

The post holder will provide specialist assessment, treatment, education, and ongoing support to patients within community settings, working closely with service users, carers, primary care colleagues, and partner organisations.

They will contribute to the planning, implementation, and evaluation of individualised care plans, ensuring care is delivered in line with current clinical evidence, national guidance, and local service priorities.

The role offers the opportunity to develop specialist expertise in diabetes management while supporting service development, delivering education and training to healthcare professionals, and participating in quality improvement, audit, and research activities.

The successful candidate will demonstrate excellent clinical, communication, and organisational skills, with a commitment to delivering person-centred care and promoting self-management to improve the lives of people living with diabetes.

  • Provide specialist assessment, treatment, and management for adults with diabetes in community settings.
  • Support and empower patients to self-manage their diabetes and improve health outcomes.
  • Deliver expert advice on diabetes treatments, including oral medications and insulin therapy.
  • Independently assess, plan, implement, and evaluate personalised care plans.
  • Work collaboratively with multidisciplinary teams and primary care partners to coordinate care.
  • Manage referrals and provide timely specialist support for patients with complex diabetes needs.
  • Educate and support patients, carers, families, and healthcare professionals.
  • Contribute to service improvement, clinical audit, research, and quality initiatives.
  • Promote and implement evidence-based practice, clinical standards, and guidelines.
  • Supervise, mentor, and support colleagues, students, and wider healthcare staff.
  • Maintain accurate and comprehensive clinical records.
  • Uphold professional accountability in line with the NMC Code and engage in continuous professional development.
