We are seeking a compassionate and dedicated ASW - Social Worker to join our Hospital Pathway and Reablement team at Grade 8 in the Social Care Qualification stream. This vital role involves working collaboratively within a multidisciplinary team to support patients transitioning from hospital to home or alternative care settings, ensuring they receive the appropriate level of care and support to maximize their independence and well-being.
