An excellent opportunity has become available for a highly motivated, flexible and passionate Advanced Clinical Practitioner to join the Older Adult Home Treatment teams. Applicants must hold an ACP qualification or be currently undertaking an ACP programme to be considered for this post. People currently undergoing ACP training are eligible to apply for Band 7 and people who have completed the training are eligible to apply for Band 8a.

Teams Covered:

  • Croydon Home Treatment Team based at the Bethlem Royal Hospital
  • Lambeth, Southwark and Lewisham (LSL) Home Treatment Team based at the Maudsley Hospital

Key Responsibilities:

  • The post holder will support the clinical team leader in providing clinical leadership and managerial oversight for the allocated caseload.
  • They will support transfers across the Older Adult pathways, taking a lead in clinical decision making.
  • The role involves delivering high-quality, safe and contemporary care, ensuring consistently high standards of practice and clinical excellence, including promoting population health, improving personal outcomes, contributing towards service evaluation, and leading and developing research and quality improvement initiatives.

Contract:

Contract type: 6-month fixed-term contract. A 6-month secondment will also be considered.
