6 to 12 month Contract Full time/Part Time Band 7-8a inner
The practice pharmacist must:
- provide exemplary care
- provide support and advice to the other members of the MDT
- advise the practice of the safe management of medicines
- assess the wellbeing of patients in regard to their medication
- have competency across a range of pharmaceutical subjects supported by professional and clinical knowledge
- contribute to patient safety culture
- help lead person centred medicines optimisation
- promote patient involvement in their care
- participate in regular appraisals and attend training and development activities
- maintain a non-judgemental attitude
- uphold patient confidentiality
- plan and manage and monitor and review general care plans for patients with chronic conditions
