The Crisis Resolution & Home Treatment team (CRHT) is currently looking to recruit Band 6 practitioners to join the team, covering Liverpool and Sefton. CRHT operates on a 24 hour, 7 day a week basis, providing intensive home support to service users as an alternative to hospital admission.
The post holder will be expected to work on a rotational shift basis in accordance with service demands, including weekends and night shifts. Responsibilities include:
- Assessment of care needs and development, implementation, and evaluation of programmes of care within the framework of Effective Care Co-ordination (ECC).
- Operating as part of an integrated Home Treatment Service and delivering care within various environments.
- Assessing and providing treatment to service users with complex needs, considering alternative options to admission where possible.
- Demonstrating an attitude that respects and values service users’ choice, working alongside them and their carers to ensure they are at the centre of care planning.
- Participating in a 24hr roster system, 7 days a week, on a rotational basis.
- Conducting face-to-face assessments, specialist mental health assessments, and supporting the wider police community.
- Delivering training and education to police colleagues and signposting to appropriate partner agencies.

Key responsibilities also include:
- Formulating, contributing to, and participating in the development and implementation of assessment and care planning systems in collaboration with the multidisciplinary team.
- Managing a case load within the Crisis Resolution Home Treatment Team.
- Carrying out generic and specialist assessments, including risk, and providing therapeutic interventions/home treatments based on service users’ and/or carers’ needs.
- Completing gatekeeping for all inpatient admission requests across the Merseyside footprint.
- Assessing risks and collaboratively developing safety plans with users/carers.
- Ensuring effective liaison with inpatient units across the Trust to facilitate early discharges or reviews of users placed out of area.
- Acting as the Duty Practitioner/2nd Duty as part of a rotational roster.
- Participating in ECC and undertaking a care-coordinator role as appropriate.
- Assisting in the development and implementation of integrated care pathways, including appropriate service user assessments/referrals and models of care.
- Ensuring record keeping is in accordance with Professional and Trust standards.
- Ensuring the safe management of medicines and adhering to safe practice and principles of administration in accordance with the NMC code of professional conduct and Trust Medicines Code.
- Working in a professional and collaborative manner with the multi-disciplinary team.
- Participating and promoting positive intra-agency working.
- Acting as a knowledge resource in specialist areas of practice to other team members and agencies as required.
- Where indicated and following relevant training, acting as a non-medical prescriber, working within their sphere of clinical competence.
