Perform independent specialist assessment of patients to formulate a diagnosis, plan and implement care and arrange ongoing follow up as an advanced practitioner using highly developed specialist clinical knowledge and advanced clinical skills.
Independently manage a patient caseload providing expert specialist assessment, planning and evaluation.
Lead the multi-disciplinary team in resolving patient problems by the provision of new and innovative models of care.
Act as a role model and expert clinician providing in-depth specialist clinical knowledge to colleagues through MDT meetings, for patients and carers/relatives.
Provide leadership, guidance and advice to staff on operational and professional issues promoting an open, honest and transparent culture.
Improve the experience and outcomes of care for patients by taking the lead in investigations and treatments prescribed.
Provide a consistent and personalised service by ensuring that the patients receive the best care, in the best place from the best person or team.
Provide social, emotional and psychological support for patients from referral and throughout their treatment.
Deliver expert specialist evidence-based care including: development, assessment, planning, implementing, evaluating programmes of evidence-based nursing care for a defined caseload.
Make appropriate referrals to consultants/other specialists/members of the multidisciplinary team in primary, secondary or tertiary care settings.
Be aware of risk management issues both locally and nationally and develop strategies to address identified issues.
