Lincolnshire Community Health Services are seeking 2 x full time District Nurse to join them within the Lincoln North Community Nursing Team. The team are based in Lincoln. Applicants should hold the Community Specialist Practitioner qualification and have experience of managing a caseload of patients and leading a team of staff in delivering excellent patient care. Alternatively, a Case Manager without the DNSPQ may be considered for the right candidate on a temporary basis with the expectation that the course is undertaken when available.

The service delivers care to patients in a variety of community settings across a 7 day service covering the hours of 8:00-18:30 (shift work involved). The District Nurse/Case Manager will hold a caseload of patients with varying health needs, coordinating care delivery. They will need to have extensive communication, interpersonal and clinical skills in order to be successful.

The District Nurse/Case Manager is essential to improving the quality of care that we deliver and in ensuring that the patient has a positive experience.
