  • Perform comprehensive assessment of patients' chronic pain needs, plan, implement and evaluate care delivery in an outpatient setting.
  • Enable the delivery of a high standard, specialised chronic pain management provision to all Trust sites and outpatient clinics.
  • Manage and organise own caseload of patients within various settings using own initiative with the support of other team members.
  • Perform clinical skills in the outpatient environment providing assessment & treatment of chronic pain using guidelines to enhance the patient/carer experience.
  • Collect, collate, evaluate and report information, maintaining accurate patient records in line with clinical governance and risk management.
  • Involve patients and carers/relatives in the planning and delivery of care and development of the chronic pain service to enhance the patient/carer experience.
  • Communicate sensitive & complex information to patients/carers/relatives and professionals across primary and secondary care regarding treatment plans & care decisions throughout the patient journey.
  • Work collaboratively with other professionals and members of the multi-disciplinary team to ensure patients' pain needs are met, especially in relation to ongoing care needs.
  • Act as a role model in the delivery of care to patients.
  • Use leadership and team building skills to ensure high quality of care is provided to all chronic pain patients.
  • Demonstrate a sound research-based knowledge of chronic pain management therapies and conditions.
  • Undertake audit and evaluate service delivery and clinical effectiveness as required.
  • Ensure compliance with research governance, locally and nationally.
  • Help in the development of databases and use relevant software in audit to facilitate audit.
  • Maintain personal professional knowledge and understanding of current issues through commitment to self-development through lifelong learning.
