The successful candidate will be based in Leigh PCN comprising of 10 GP surgeries. We are looking to recruit two Care Coordinator to join our PCN team. The job requires a caring, dedicated, reliable, person-focused candidate who enjoys working with a wide range of people. They will have good organisational and time management skills, have data collection abilities and be able to work on new projects. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.
