Why am I here? (Purpose) To support the safe, effective delivery of face-to-face and virtual pulmonary rehabilitation across South and South East Staffordshire, helping people with respiratory conditions improve their health and self-management while providing responsive patient communication and essential administrative support to the multidisciplinary team. What am I aiming to achieve? (Outcomes) Deliver safe, effective face-to-face and virtual pulmonary rehabilitation in line with agreed plans, protocols and competencies. Support patients to improve their health, wellbeing and confidence in self-managing respiratory conditions. Monitor progress and accurately complete outcome measures, patient feedback and pre- and post-programme assessments. Maintain timely, accurate clinical records, correspondence, referrals, appointments, discharges and audit data. Communicate effectively and respectfully with patients, carers, colleagues and partner services while protecting confidentiality, dignity and safety. Any reasonable duties shall be undertaken to achieve the above. How I will know I have succeeded? (Success Measures) Pulmonary rehabilitation sessions are delivered safely, consistently and within agreed protocols, competencies and delegated plans. Patients complete pre- and post-programme assessments and demonstrate measurable progress, including relevant outcome measures and MCID reporting. Patients receive clear advice and motivational support that improves confidence in self-care and management of respiratory conditions. Clinical records, appointments, referrals, discharges, correspondence and audit data are accurate, complete and timely. Patient feedback and programme audit information are captured and used to evaluate and improve service delivery. Communication with patients, carers, colleagues and partner services is responsive, respectful, confidential and adapted to individual needs. What will I do? (Responsibilities) Implement delegated pulmonary rehabilitation plans under indirect supervision, working within agreed competencies, protocols and professional boundaries. Lead and assist face-to-face and virtual group exercise, education and advice sessions, making minor protocol-based adjustments and escalating concerns appropriately. Monitor patient progress, complete pre- and post-programme clinical outcome measures and PROMs, and report findings to the registered practitioner and GP as required. Use motivational approaches to support self-care, self-management, health and wellbeing for individuals and groups. Act as a first point of contact, respond sensitively to enquiries, record messages and arrange face-to-face or virtual assessments and consultations. Use the Spirit clinical information system to retrieve information, manage referrals, tasks and discharges, and produce and send correspondence. Maintain accurate, contemporaneous written and electronic clinical records and share information appropriately with relevant professionals. Capture patient evaluations, process statistics and contribute to local and national pulmonary rehabilitation audits and service evaluation. Communicate effectively with patients, carers, colleagues and partner organisations, adapting methods to overcome barriers to understanding. Protect privacy, dignity, consent, confidentiality and data protection, and contribute to safeguarding, clinical risk management and incident review. Organise and prioritise the delegated workload, maintain quality, participate in supervision, mandatory training, CPD and shared learning, and remain within own competency. Maintain awareness of relevant pulmonary rehabilitation and respiratory standards, guidelines and evidence-based practice.
