LOROS Hospice in Leicestershire is seeking a Discharge Liaison Nurse to join the Hospice Discharge Team.
You will coordinate safe, timely discharge from inpatient care, working with MDT colleagues to plan care packages and liaise with external agencies to secure continuing care funding.
Your role includes direct patient contact, contributing to discharge planning, maintaining records, and upholding safeguarding and infection control policies.
