Profile: The post holder is part of the Hospice Discharge Team, whose aim is to enable safe and timely discharge of inpatients from the hospice.
The post holder is responsible for: The safe and effective discharge of patients, in line with current legislation, policies and procedures.
Work in collaboration with the other members of the multidisciplinary team (MDT) to coordinate and Facilitate discharge to home, care home or another hospice or hospital by Carrying out Continuing Care Assessments and liaising with external agencies such as Continuing Healthcare Commissioning Support Units, Leicestershire Partnership Trust, University of Leicestershire Hospitals, and Adult Social Care.
Clinical Duties include
- To be responsible for assessments as necessary to facilitate discharge and packages of care for patients in accordance with Leicester, Leicestershire and Rutland Health needs assessment performed under continuing care legislation.
To ensure that discharge is timely and appropriate.
Ensure that the patients and their carers are kept fully informed of all discharge arrangements.
Establish links and close working relationships with the Clinical Commissioning Groups who co-ordinate access to continuing care funding.
Have effective working relationships with the wider MDT.
- Maintain personal direct contact with individual patients.
- Liaise with and assist the medial staff in the treatment, care, well-being and safety of the patient especially relating to discharge.
- To carry out and comply with the prevention and control of infection as per LOROS policies and procedures.
- Represent the Discharge and Liaison Team at MDT meetings, contributing to the planning of patient discharge.
- Have an awareness of Safeguarding policies and procedures, and professional responsibilities to elevate concerns.
Educational and personnel duties include
- To participate in the education of all staff on topics related to continuing care.
- Assisting where appropriate with the induction and support of newly appointed trained, untrained and volunteer staff.
- Supervision and assistance in the teaching of other nursing staff and volunteers in the Hospice.
- Generating and maintaining a friendly and supportive team spirit with all Hospice staff.
- Attend internal training, conferences and study days.
Administrative duties include
- Maintain accurate records of all discharge documentation and provide data as requested for monitoring purposes.
- Be aware of the Health and Safety at Work Act, ensuring that related procedures are carried out correctly and recording accidents to patients, staff and visitors.
- Follow Hospice procedure for maintenance and repairs of equipment.
- Be aware of any complaints, taking immediate action and escalating in accordance with Hospice policy.
- Maintain confidentiality of patients and relatives.

All staff are subject to LOROS policies, procedures and conditions of service, with reference to the Health and Safety at Work Act 1974.
The post holder is responsible for safeguarding the interests of children and adults who they come into contact with during their work.
To fulfil these duties post holders are required to undertake mandatory training to recognise the signs and symptoms of abuse, or neglect, or individuals at risk and to follow LOROS policies relating to safeguarding practice including reporting any concerns that they may have.
Take reasonable care for the health and safety of themselves and of other persons who may be affected by their acts or omissions at work.
Co-operate with their employer as far as is necessary to meet the requirements of the legislation.
