ADHD Single Point of Access Pathway Navigator

The Single Point of Access Pathway Navigator will: Complete comprehensive and holistic assessments of need for patients with suspected ADHD who fit the criteria for the service within agreed timeframes.

Identify patient safety risks and escalate concerns when necessary to the appropriate person, following correct protocols and within a timely manner.

Develop collaborative wellbeing action plans with service users with ADHD symptoms which promote choice and accessibility.

Discuss all available providers of ADHD assessments enabling clinically appropriate and informed patient choice.

In cases where it is agreed to refer, all necessary patient administration will be completed as per agreed protocols.

Identify any barriers to people accessing support for their ADHD symptoms and work with individuals to overcome these.

Use a range of techniques and interventions to support people with ADHD symptoms, to better understand themselves from a strengths and challenges perspective, to encourage a more positive mindset and ability for self-advocacy.

Monitor and record outcomes within agreed clinical templates, assessment tools and outcome measures.

Reflect on personal and team practice, participating openly and constructively in team meetings and huddles, peer supervision, and MDT meetings Ensure that patient records and other information systems are completed accurately and within agreed timescales.

Responsible for reporting incidents accurately and completing all required training and learning within deadlines.

Ensure timely and comprehensive preparation for MDT meetings, with all required patient information collated, updated, and shared in advance (as per the MDT standard operating procedure).

Maintain high standards of patient care, evidenced through positive patient feedback and the timely, effective management of patients across service pathways.

Achieve agreed service activity, quality, and outcome targets in line with contractual KPIs and organisational standards.

Worked within agreed KPIs, escalating issues promptly when targets cannot be met.

Ensure that support undertaken is meaningful and respectful, promoting self-understanding and self-compassion, and is in line with the individual action plan.

Adhere to organisational policies and procedures relating to risk and patient safety.

Work with team members and colleagues to continually improve the quality of services within the overall organisational governance frameworks and service objectives.

Have good written and verbal communication skills and adapt their approach for people with ADHD as required.

Have awareness and experience of working with and respecting confidential material.
