Community Nurse (Mental Health and Learning Disabilities) Tri-borough - Hammersmith and Fulham, Kensington & Chelsea and Westminster. The job role includes the following:

  • To provide specialist nursing support to people with a Learning Disability (LD) at risk of breakdown or admission to mental health hospital for assessment and treatment on the Dynamic Support Register.
  • To provide specialist nursing support to clients at risk of placement breakdown due to mental health difficulties.
  • As part of a team to review Section 117 aftercare plans for clients known to the ICB residing within the three boroughs.

We are looking for:

  • a clinician with experience of, or a strong interest in, working with people with learning disabilities and mental health challenges
  • a clinician who keeps the client at the centre of their practice.
  • a dedicated motivated individual who has the experience to work independently within a multidisciplinary team.

The main duties of the job are supporting clients with learning disabilities and mental health difficulties in the community who are at risk of admission or placement breakdown. You will be working closely with the client and their support network. You will be expected to complete assessments relating to mental health and learning disabilities including identifying strengths and risks and developing care plans and crisis and contingency plans reflective of the clients needs.

As part of a team you will manage a small caseload of clients who require a yearly review of their section 117 aftercare plans. The aftercare of these clients is held by the ICB and the clients reside within the team catchment area. You will work closely with the Consultant Psychiatrists providing mental health support and liaise with the wider Community Learning Disabilities team including nurses, psychologists, speech and language therapists and occupational therapists.

We are looking for someone who puts the client at the centre of their practice and has experience of:

  • working with people who have a learning disability and/or working within mental health
  • understanding of the Dynamic Support Register
  • Understanding of Section 117 after care review process.

The LD Psychiatry service works across three boroughs offering the psychiatry input into the Tri-Borough LD Community Team. This post offers the flexibility to be based in Hammersmith & Fulham or Westminster. The role requires travel around the three boroughs. We are looking for a clinician who is able to work independently and to organise their time effectively. You would receive clinical supervision from the Trust Consultant Nurse for Learning Disabilities and be part of the active Learning disabilities and Autism Clinical network. Management supervision would be provided by the LD Psychiatry service manager.

Your role will be to provide Health Case Management/named worker specifically for ICB responsible clients who are at risk of placement breakdown or admission and/or on the DSR. The role of case manager is multidimensional, requiring a unique blend of clinical knowledge, people skills and administrative abilities. The primary function of this role is to support people who are at risk of placement breakdown or admission to mental health hospital for assessment and treatment or face placement breakdown due to mental illness / behaviours of concern including the creation of crisis and contingency plans.

You will be part of a team responsible for reviewing S.117 aftercare plans, alongside other involved services, of Tri-Borough patients, including those in placements outside of the Tri-Borough but within the NWL Commissioning area. This involves following a review process and creating aftercare plans to support the client. You will work within the multi-disciplinary team structure of the Tri-Borough Community LD Services, providing specialist professional advice and expertise in learning disabilities for this cohort of patients, this can include supporting clients when medication is initiated within case load numbers.

The community LD service consists of nursing, psychology, speech and language therapy and occupational therapy. The post-holder will establish and maintain a high standard of care, ensuring that the quality of care provided is continually informed by the best evidence-based practice.
