A vacancy has arisen within the Triage Team for the West Place and to support unplanned nursing care and catheter clinics in the community.

The applicant should ideally have excellent communication skills with a good understanding of IT systems.

Previous Community Nursing experience would be beneficial for this position.

We are looking for a well-motivated team player who will be expected to work in a busy environment and able to meet the demands of this hard-working service.

Support and any additional training would be provided to ensure competency and skills are maintained.

The funding for this post is for 7 months at present, this is likely to extend or become substantive.

To triage incoming referrals assessing the patient’s needs, determining the priority of care and processing the referrals accordingly.

To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by assessing, planning, implementing and evaluating packages of care.

The position will require visiting patients in their own environment to provide unplanned nursing care.

We are a small team made up of Triage Nurses, allocators and led by a Clinical Operations Manager.

We endeavour to provide excellent triaging to ensure that safe, relevant and timely care is provided by the community nursing teams to the local housebound population of West Norfolk.

We work closely with our GP and Social Services colleagues and visit patients in their own homes, care homes and clinics as appropriate.

  • Key Areas of Responsibility
  • To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.
  • To document triage notes in order that the patient’s record is complete and for audit purposes.
  • To use information technology to process the referrals for the 80% triaging role.
  • To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
  • To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
  • To undertake unplanned care in conjunction with community nursing teams.
  • To support community nursing teams with provision of care in catheter clinics
  • To work within the integrated team to facilitate early discharge from hospital.
  • To work within the integrated team to prevent unnecessary admission to hospital.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
