Junior sister - Community Children's Nurse | East London NHS Foundation Trust

We are looking for an enthusiastic children's nurse to work within our children's community nursing team.

We deliver (in person) community based care to children under 6 nursing pathways:

(1) Short term (wound care/IVABS); (2)Acute - Oncology, home oxygen therapy, pre term babies; (3) Long term conditions; (4) Complex disabilities; (5) Palliative; (6) End of Life care

Our team works across London Borough of Newham, supporting babies, children and young people, from pre term to 19yrs old, with health needs in the community settings (home, schools and clinics).

We are based within East London, and work as part of an Multidisciplinary team, including play, psychology, dietetics, as part of our community nursing service.

We support professional development (including clinical skills within the role) and staff well being.

  • To manage a clinical caseload ensuring continued responsibility for the assessment, planning, development, implementation and evaluation of programmes of care.
  • To work across community children’s nursing service (CCNS) nursing pathways.
  • To work in partnership with other community/ acute based teams, clients and services in the public, private, educational and voluntary sectors.
  • To be able to provide line management for junior members of the team associate to caseload, including clinical supervision and appraisals.
  • To demonstrate areas of specialty thereby acting as a resource for other professionals and enhancing evidence-based practice for delivery of community based nursing care for children and young people with a variety of conditions, including life limiting and complex care..

  • Provide high quality nursing care for children and young people in the London borough of Newham, which is planned and delivered, reviewed and evaluated within the clients home, wound dressing clinic or other appropriate setting within the community using family centered, evidence based practice.
  • Assess and order equipment and supplies for the child or young person to enable them to manage their condition safely in the community setting, including regular reassessments dependent on the changing needs of the child/young person.
  • Provide initial triage and assessment of new referrals to the service, identifying where urgent response is required and allocating to staff, based on staff logistics and skill mix.
  • Initiate and manage safe, effective discharge from hospital to home carrying out risk assessments of the receiving community setting.
  • Enable and promote independence by teaching, supporting and encouraging carers and children, including those children with complex health needs, to participate in meeting their own care needs where possible in community settings.
  • Initiate and manage appropriate changes in care in negotiation with children, families and professionals following review of planned care at either case review, morning handover or joint visits.
  • Provide emotional support to clients and families who may be facing difficult decisions, bad news or bereavement.
  • Communicate with families for whom English is not their first language and/or children with communication difficulties/ special needs.

This involves working in collaboration with health advocates (interpreters), or using highly developed verbal/ non-verbal communication skills
