Primary Homecare, rated outstanding by CQC are looking to recruit a professional full time Coordinator to join their friendly team.
To work closely with the Registered Manager to ensure that quality care is tailored to each person’s needs and delivered efficiently in a manner that reflects the values of Primary Homecare.
To oversee and manage a team of Home Care staff. Cover the care as required i.e. in times of absence or staff shortages.
Complete client assessments and develop the individualised Care Plan.
To ensure that all new staff are supported to assist them to settle in their new role
To ensure that the services provided comply with contractual obligations and other legal requirements as detailed in specifications and contract agreements specifically the Support to Live at Home contract
To make initial contact with social workers, clients and their representatives in order to carry out assessments and ensure care plans are produced and made available for all members of the care team
To ensure that all Health and Safety considerations are understood and implemented according to legislation.
To ensure that staff rotas and work schedules are designed to meet clients’ needs and are produced and recorded on Nurse Buddy System
To provide support and supervision to the care team on a two monthly basis to contribute to their development needs as spot checks, workplace supervisions and face to face meetings including chairing staff meetings.
To undertake or ensure that regular reviews of service user needs are carried out as and when required to meet their changing needs.
To implement the necessary procedures to ensure that the required documentation is completed for all service users, and processed in accordance with company policy.
To ensure that appropriately trained staff are available to respond to calls for assistance from clients. Deliver Training Sessions as required
To provide and participate in an on call system to ensure that the care team and clients/representatives can contact an agency representative for advice and guidance when the office is shut. To provide “hands-on” coverage if no other care workers are available in all areas that we provide a service in. To ensure that on-call records are recorded clearly and precisely and made available for the Registered Manager. A clear hand-over must be adhered to from the on-call Co-ordinator to the office team to ensure the smooth running of the company. Any instances that you are unclear of must be reported to the Registered Manager for advice
