The Integrated Wellbeing and Bereavement Service Coordinator plays a key role in delivering compassionate, person-centred support to patients, families and carers from diagnosis through illness and bereavement. You will work within the Integrated Wellbeing and Bereavement Team, coordinating holistic support to help people live well, navigate loss and access appropriate care at the right time.
This role involves managing an allocated caseload, collaborating with colleagues, volunteers and partners.
