This is an exciting opportunity for an Integrated Rehabilitation Support Worker (Band 3) to join our multidisciplinary team at Athlone Rehabilitation Unit. In this role, you will help deliver high-quality, safe and effective rehabilitation that is person-centred and focused on supporting patients to achieve their goals.

You will work as part of a multidisciplinary team (MDT) to provide coordinated care within our bedded rehabilitation services. We welcome applicants from a wide range of backgrounds and experiences. You will work closely with registered therapists, nurses and other team members to provide responsive, supportive care that meets each person’s individual rehabilitation needs. Clear communication within the team will be essential to ensure patients receive consistent and coordinated care. You will support a positive patient experience by promoting independence, encouraging participation in rehabilitation goals, and working in partnership with families and carers. This role requires a compassionate and respectful approach, always maintaining dignity, choice and inclusion for every individual.

In this role, you will:

  • Manage a defined caseload, working to agreed rehabilitation goals and supporting qualified staff to deliver individual treatment programmes
  • Deliver therapy and nursing interventions within your agreed scope of practice, as part of the multidisciplinary team
  • Complete required competencies with support and assessment from qualified staff
  • Communicate patient progress, outcomes and any concerns clearly to the relevant therapist or nurse
  • Support patients to develop independence in daily living skills, including personal, social and functional activities
