Derbyshire Community Health Services NHS Foundation Trust is seeking a SCPHN Health Visitor to join the Health Visiting Team.

You will deliver the Healthy Child Programme to the 0-5 population and their families, developing therapeutic relationships even in complex family situations.

You will manage a defined active caseload, address Safeguarding Children issues, and work with partner agencies and the voluntary sector to improve local health outcomes.
