We are looking for an enthusiastic and highly motivated clinical pharmacist to lead our Specialist Medicines Pharmacy Service. You will provide highly specialist and advanced clinical and professional pharmaceutical services to patients at Newham Hospital. The successful candidate will join a supportive, integrated pharmacy team, committed to delivering high quality patient care. You will have access to further educational opportunities and senior clinicians within Pharmacy and other disciplines who can provide mentorship and aid professional development.
Roles and responsibilities include (but are not limited to):
- To provide a highly specialised clinical pharmacy service to the Specialist Medicine Service department as part of the multidisciplinary team.
- To provide clinical pharmacist ward cover as per the rota.
- Optimisation of medication for the individual patient ensuring medicines reconciliation, appropriate clinical screening as well as discharge support via drug listing.
- To provide clinical advice and care to specialist medicine inpatients when required.
- Provision of homecare prescription management.
- Contribute to virtual biologic clinics and other multi-disciplinary meetings.
- Development of evidence based prescribing guidelines.
- To manage a Pharmacist-led specialist medicine Clinic.
- To prescribe medication within scope of practice.
- To develop external links, local primary care organisations and any other relevant local and national specialist interest groups.
- To promote high-quality, cost-effective prescribing for patients.
- To provide the Associate Director of Pharmacy and Clinical Services Manager with details of output, performance and activity at specified time intervals in an agreed format.
- To lead on the implementation and continuation of biosimilar switch schemes and ensure that patients are switched within the required timeframe.
