We provide short-term health, therapy and care support to help people remain safely at home, preventing unnecessary hospital admissions and supporting timely discharges. Available 24/7, 365 days a year, our teams deliver compassionate, person-centred care to vulnerable patients, helping them regain independence and achieve the best possible outcomes in their own homes.
This is an exciting time to join us as we expand innovative services, including Virtual Wards, enabling patients with conditions such as COPD and respiratory infections to receive specialist care and remote monitoring at home. From urgent care and rehabilitation to discharge support and end-of-life care, our multidisciplinary teams work together to make a real difference every day.
If you are looking for a rewarding role where you will play a vital role in supporting patients to remain safe, well and independent within their own homes and community settings. Working as part of a multidisciplinary team, you will deliver high-quality, person-centred care, ensuring care is provided in line with agreed care plans, risk assessments and professional standards.
You will provide practical and compassionate support, including personal care, meal preparation and medication administration, while carrying out clinical observations and delegated procedures. A key part of the role is recognising changes in a patient’s condition, escalating concerns appropriately and providing timely feedback to senior clinicians.
You will also support the triage of referrals, undertake basic and follow-up assessments, develop care plans and risk assessments, arrange equipment and make onward referrals where required. In addition, you will gather patient feedback to support service improvement and maintain a strong understanding of Trust policies and procedures to promote patient safety and quality care.
