Join our Urgent Care Services team and play a vital role in helping people stay safe, independent, and well in their own homes. Our service provides short-term health, therapy and care support to individuals who may otherwise require hospital admission, while also supporting timely and effective hospital discharges back into the community.
No two days are the same. You'll work alongside experienced nurses, therapists and healthcare professionals, delivering high-quality, person-centred care that has a direct impact on patients' lives. From supporting recovery and promoting independence to helping people regain confidence after illness or injury, you'll be part of a responsive and compassionate team dedicated to keeping care closer to home.

As a valued member of our multidisciplinary team, you'll deliver delegated aspects of patient care under the guidance and supervision of registered nurses, therapists and healthcare practitioners. You'll support the assessment, implementation and review of care plans by monitoring and reporting patient progress, helping to ensure individuals receive the right care at the right time.
Your varied role will include undertaking clinical activities that you have been trained and assessed as competent to perform, such as:
- recording vital signs
- glucose monitoring
- routine urinalysis
- wound care
- falls prevention support
- equipment provision
- assisting patients with activities of daily living
You'll help people build confidence and independence, delivering both individual and group interventions where appropriate. Working within established protocols and care pathways, you'll contribute to safe, effective and compassionate care, supporting patients to achieve the best possible outcomes while remaining within their home and community whenever possible.
Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026. Following integration and to align our community services to the new Neighbourhood Model of care delivery, there will be a period of consultation which may result in changes affecting some services, teams or roles.
