Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Are you an experienced and motivated Health Practitioner ready to take the next step into leadership? We are looking for a dynamic Team Lead to join our Health & Social Care Coordinator (HSCC) and Primary Care Lifestyle Teams within East Kent Long Term Services. This is a rewarding and varied role where you’ll blend clinical expertise with leadership, making a real difference to patient outcomes. You will lead and support staff through supervision, mentoring, and line management, while coordinating service delivery across both teams.

Alongside leadership responsibilities, you’ll remain hands‑on in patient care — carrying out assessments, supporting care coordination, contributing to MDT working, and guiding patients to the right services. You’ll also undertake Trusted Assessor equipment assessments (full training provided).

Working closely with Community Nursing teams, GPs, and wider partners, you’ll play a key role in reducing health inequalities, improving quality of life, and preventing unnecessary hospital admissions. You bring a strong clinical background, combined with leadership or supervisory experience, and a passion for delivering integrated, patient‑centred care. You’re highly organised, a confident communicator, and thrive in collaborative environments.

Key responsibilities include:

  • Leading, mentoring, and supporting staff development
  • Coordinating and overseeing service delivery across both teams
  • Monitoring resources, budgets, and contributing to service improvement
  • Collecting, analysing, and reporting data to support outcomes
  • Assessing patient needs and facilitating appropriate referrals
  • Communicating effectively with MDTs and senior management

This role is based across Canterbury and Ashford, with regular travel for home visits, clinics, and multidisciplinary meetings. If you’re ready to lead with purpose and improve patient care across East Kent, we’d love to hear from you.
