An exciting opportunity has arisen for an experienced nurse to join the Specialist Palliative Care Team based at Northwick Park Hospital as a Band 7 Clinical Nurse Specialist. You will be part of a dynamic team that strives to provide a responsive and high-quality Specialist Palliative Care Service to our patients and colleagues, supporting each other through team learning and supervision.

The Specialist Palliative Care Team provides advice, information, education, and support to adult patients and their families during the last year of life. The team provides a day-to-day specialist palliative care nursing resource across the Trust and is supported by an established team of senior nurses and medical clinicians. It also contributes to the delivery of End of Life Care education and training for Trust staff.

The post holder will have a strong clinical role evaluating individual patient needs including assessment, history taking, and treating patients. This role has responsibilities across all clinical and educational sites within the Trust.

  • To provide a timely and responsive service for patients referred to the palliative care service.
  • To assess, plan, and manage a caseload of patients and their families in the field of palliative care working in partnership with other care providers.
  • To identify, develop, and monitor innovative opportunities for education and training for Trust staff regarding the care of patients within the last year of life.
  • To identify and establish opportunities to promote more effective multidisciplinary working and facilitate education towards improved care.
  • To work autonomously and seek collaborative relationships with others across and within professional boundaries.
  • To develop patient pathways and be responsible for developing the nursing service with key colleagues.
  • To manage resources effectively including budget, human, financial, and physical.
  • To provide expert knowledge and advice on specialist palliative care, ensuring maintenance of clinical excellence underpinned by evidence-based practice.
