Highly Specialist Social Worker | London North West University Healthcare NHS Trust
We are looking for an experienced, qualified social worker to support the timely transition of patients with complex neurological disabilities from hospital to community following rehabilitation.
The role is based in the Regional Hyper‑acute Rehabilitation Unit (RHRU), a 24‑bed specialist neuro‑rehabilitation service that offers intensive, multi‑disciplinary care to adults with severe and complex cognitive, communicative, physical and psychosocial difficulties.
Responsibilities
- Assess patients and support their transition from hospital to community.
- Manage own caseload, working in close collaboration with the multidisciplinary team and external agencies to ensure complex care plans and rehabilitation/maintenance therapy are set up to meet ongoing needs in the community.
- Serve as a key worker supporting patients and their families through the rehabilitation journey, liaising with the multidisciplinary team and external agencies.
- Provide professional leadership in situations with highly complex case arrangements, including issues involving capacity, deprivation of liberty (Do L), risk, complexity and safeguarding of the patient or family members.
- Offer specialist advice relating to the National Framework for NHS Continuing Healthcare/NHS Funded Nursing Care, guiding teammates to complete referrals and assessments within specified timelines.
- Manage and supervise the RHRU’s Discharge Coordinator and Information & Benefits Advisor.
- Address any complicated issues that may arise on a day‑to‑day basis and, if required, support the multidisciplinary team in emergency situations.
Qualifications
- Qualified social worker with experience in neuro‑rehabilitation and complex case management.
- Excellent organisational and negotiation skills.
- Knowledge of health and social care pathways and assessment tools.
- Experience in mental capacity and safeguarding, with a good understanding of deprivation of liberty policies.
- Ability to liaise effectively with a wide range of external agencies across the South East of England.
