Are you a compassionate, dynamic nurse with a passion for community healthcare? We’re looking for an enthusiastic District Nurse Deputy Leader to join our collaborative and supportive team in Harrow. In this role, you will be an integral part of our Integrated Care Services, working closely with patients, families, and a multi-disciplinary team to deliver high-quality, person-centred care in the community.

  • Work collaboratively within the Integrated District Nursing Team and wider multidisciplinary team to deliver high quality, coordinated, and patient centred care across the community.
  • Assess, plan, implement, and evaluate care in partnership with patients, families, and carers, ensuring personalised care plans reflect individual needs and preferences.
  • Support the delivery of safe, effective, and responsive community nursing services, promoting independence, wellbeing, and positive patient outcomes.
  • Contribute to service improvement and quality initiatives, supporting the ongoing development of community services to meet the changing needs of the local population.
  • Deputise for the Team Leader as required, providing day to day clinical and operational leadership and ensuring continuity of service delivery.
  • Support the supervision, development, and wellbeing of team members, promoting a positive learning culture and high standards of clinical practice.
  • Work in partnership with primary care, acute services, social care, and other healthcare providers to ensure seamless care pathways and effective multidisciplinary working.
  • Contribute to clinical governance, risk management, and quality assurance activities to support continuous service improvement and excellent patient care.
