The predicted salary is between 31500 - 38500 £ per year.

Please note that the advertised salary for this position is subject to the pending 2026/2027 national pay award. The final salary will be adjusted accordingly once this has been agreed. Newly Qualified Social Workers will commence on Grade 9 pay and job description before progressing to grade 10 after 2 years in post. Experienced social workers will commence at Grade 10 pay and job description. This position qualifies for a market supplement payment of £200 per month. Please note this additional payment will be reviewed on 31 March 2027. The successful candidate may be eligible to up to a maximum of £8,000 (including VAT) towards the cost of expenses incurred due to relocating to Ceredigion.

We’re looking for a dynamic and motivated Social Worker to join our Integrated Triage and Assessment Team. This is a unique opportunity to be part of a forward-thinking service that works collaboratively to deliver timely, person-centred support to adults and their carers.

As a key member of our team, you will:

  • undertake strengths-based assessments with adults and carers
  • develop and implement care and support plans that promote independence, wellbeing, and recovery
  • work closely with health, housing, and community partners to ensure a holistic and integrated approach
  • focus on early intervention to prevent escalation of need and support people to live the lives they choose

What we’re looking for:

  • a qualified and registered Social Worker (Social Care Wales registration required)
  • experience or interest in working with adults
  • strong communication and decision-making skills
  • a commitment to promoting independence, dignity, and choice
