WHAT DO YOU DO?

• Patient-Focused Clinical Care

  • Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
  • Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
  • Support safe prescribing through reconciliation, clinical screening, and dose adjustment (e. g., renal or hepatic impairment).

• Prescribing and Medicines Management

  • Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
  • Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
  • Participate in antimicrobial stewardship programmes and assist with monitoring high-risk medications (e. g., anticoagulants, cytotoxics, opioids).

• Multidisciplinary Teamwork

  • Act as a key clinical advisor within ward rounds, MDTs, and discharge planning meetings.
  • Provide medicines-related support and education to medical, nursing, and allied health professionals.
  • Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.

• Safety, Governance, and Compliance

  • Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.
  • Support audits, inspections, and clinical governance activities related to medicines management.
  • Contribute to policy development, clinical guidelines, and updates to the hospital formulary.

• Education and Development

  • Educate patients and staff on safe medication use, including self-administration programmes and discharge counselling.
  • Provide training to junior pharmacists, pre-registration trainees, and clinical staff on new therapies, prescribing guidelines, and pharmacy services.
  • Keep up to date with emerging evidence and clinical best practice through CPD, professional networks, and literature reviews.

WHAT DO YOU HAVE?

  • Essential: A masters degree in pharmacy (MPharm or equivalent).
  • GPh C registration as a Pharmacist.
  • Previous experience in a clinical hospital pharmacy role.
  • Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
  • Excellent communication and clinical decision-making skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Confident using electronic prescribing and medication administration systems (e. g., EPMA).
  • Desirable: Postgraduate clinical pharmacy qualification or working towards (e. g., Clinical Diploma or Independent Prescribing).
  • Experience in a specific clinical specialty (e. g., surgery, oncology, cardiology, critical care).
  • Experience in private healthcare or working to CQC standards.
  • Participation in audit, service evaluation, or quality improvement projects.
  • Training or mentorship experience.
