As a Band 5 Community Staff Nurse, you will join our valued Swanley and Rural PCN Community Nursing Team in North Kent. This is a rewarding opportunity to deliver high-quality nursing care where it matters most, within patients' own homes and local communities. You will support people with a wide range of healthcare needs, helping them manage long-term conditions, recover from illness and maintain their independence while avoiding unnecessary hospital admissions.

This is a full-time position working 37.5 hours per week. The service operates seven days per week between 8:00am and 8:00pm. Main Responsibilities:
- As a Community Staff Nurse, you will assess, plan, deliver and evaluate nursing care for patients within their own homes and community settings.
- Working closely with Senior Sisters, multidisciplinary colleagues, patients and carers, you will help deliver safe, effective and evidence-based care tailored to each individual's needs.
- You will manage your workload independently while contributing to the wider team, ensuring patient care is prioritised appropriately and any changes in a patient's condition are identified and escalated promptly.
- You will also play an active role in health promotion, service improvements, clinical audits and the ongoing development of community nursing services.
Person Specification:
- Essential: Registered Nurse with current NMC registration, Diploma level qualification or equivalent, Evidence of continuing professional development, Strong communication and interpersonal skills, Ability to work both independently and as part of a multidisciplinary team, Excellent organisation and prioritisation skills, Commitment to delivering high-quality, evidence-based patient care, Confidence using IT systems and electronic patient records, Full UK driving licence and access to a vehicle for work purposes.
- Desirable: Previous community nursing experience, Degree-level qualification, Mentorship or teaching qualification, or willingness to undertake training, Understanding of current developments within community healthcare and integrated care services.
